My Health Summary PDF: your health history on one page with MoraHealth
MoraHealth's My Health Summary PDF puts your medications, conditions, allergies and operations in one document. How to prepare it before seeing a doctor.

My Health Summary puts the medications, conditions, allergies and past operations you keep in MoraHealth into a single PDF. It is the shortest way to explain your health history when you see a new doctor, fill in a form, or face an emergency. You can download and print it in seconds, or show it on your phone.
If you are new to MoraHealth, start with What is MoraHealth?
What is it for?#
A new doctor usually asks the same questions: Which medications do you take? Do you have any known conditions? Are you allergic to any medication? Have you had surgery before? Answering them from memory, completely, is hard; medication names and doses in particular get mixed up. My Health Summary keeps those answers ready in advance, as a tidy document.
What is in the PDF?#
| Section | Content |
|---|---|
| Personal details | Name, age, sex, height, weight, blood type, the week of pregnancy if any |
| My Medications | Medication name, dose, how often and at what times, start and end date |
| My Conditions | Condition name, its status (active, past, under control) and the date of diagnosis |
| My Allergies | Allergen, reaction, severity and date |
| Past Operations | Operation name and date |
The date it was created is printed at the top, so it is always clear how current the summary is.
Lab, radiology and pathology reports are not included in this summary; for those, use MoraHealth's report pages and test history charts. See Tracking your lab results over time.
How to prepare it#
- Complete your records. The summary is built from what you enter. Add your medications with doses and times, and your conditions, allergies and operations. More on medication and appointment records in Medications, appointments, blood pressure and glucose in one app.
- Open the My Health Summary page. Your details appear section by section; a missing section stands out here.
- Check the document with Preview PDF.
- Save it with Download. The file is named with the date and the profile name, for example "10/01/2026 - Jane Doe Health Summary.pdf".
English or Turkish#
The summary is prepared in the language you use MoraHealth in. Need it in Turkish? Switch the language to Turkish and download the summary again; the headings, medication frequencies and statuses are written in Turkish. Medication and condition names you entered yourself stay as you entered them.
Summaries for your family#
Each profile has its own summary. If you keep records for your child or an elderly parent, select their profile and download their summary. Keeping records for more than one person is covered in Profiles and sharing.
A few tips#
- Download it again after a change. When you start or stop a medication, an older printout is out of date.
- Keep a copy on your phone. In an emergency you can show it even without an internet connection.
- There is a separate option for all your data. My Health Summary is a readable document meant for your doctor. For a complete, machine-readable copy of all your records, use Download my data in Account Settings; see Is my health data safe?
MoraHealth is a health records app; it is not a substitute for medical advice, diagnosis or treatment. The summary is built from the records you entered; check them regularly for accuracy.
